Household Information/Registration Form
This form needs to be filled out only once for your household to be placed in our database, to enable you to register by mail, online, or at the office. Households currently in our database will also need to fill out this form once, to enable us to update your household information and provide you with an online account.
This form can also be used for mail-in registration. To do so, fill out the Mail-in Registration section only (bottom half of 2nd page) and send it to us with a check.
